Super Bowl Sunday is almost here, and our favorite vendor partners have teamed up with Dwellus to give away four incredible prizes during Super Bowl LX, with one winner selected each quarter. Entry is completely free—simply fill out the form below, then tune in on Super Bowl Sunday to see which square you received and watch the game to find out if you win. One entry is allowed per person, but multiple family members are welcome to participate. All entries must be submitted by 3:00 PM on Sunday, February 8th.

If we receive more than 100 entries, multiple names will be placed in each square. Each participant will be assigned numbers from 0 to 9 along the grid, and the winner of each quarter is determined by using the last digit of each team’s score and matching the square where those numbers intersect. Names will be added to the grid randomly, so please check back on Super Bowl Sunday to find your square.

Tiebreaker (required): Be sure to select the team you think will win and predict the total score of the game. In the event a square contains multiple names, winners will be determined first by who picked the correct winning team, and then by who is closest to the overall total score.
